Community Falls Team

Community Activities and Additional Support Toolkit

This resource aims to assist practitioners in a range of settings, including primary and secondary health care, social care, and the community and voluntary sector, to provide information about relevant community resources to individuals living with frailty and their families and carers.

The toolkit indicates which community activities and additional support may be helpful for individuals based on where they sit on the Rockwood clinical frailty scale. 

Connecting individuals to wider community support is worth exploring whenever possible alongside other help, as there is widespread recognition that communities have a vital role in supporting health and well-being.

Active North Yorkshire 

A new sport and active wellbeing service will be launched in September when Selby and Tadcaster’s leisure centres become the first to move under the banner of the newly formed Active North Yorkshire. The move is the first stage of our plan to bring all leisure services in-house and concentrate on improving the physical and mental health and wellbeing of our communities.
